{"id":149,"date":"2024-07-10T00:37:07","date_gmt":"2024-07-10T00:37:07","guid":{"rendered":"https:\/\/findcare.hyperstage.com.au\/?post_type=job&#038;p=149"},"modified":"2024-07-16T23:58:20","modified_gmt":"2024-07-16T23:58:20","slug":"care-manager-12-months-maternity-leave-contract","status":"publish","type":"job","link":"https:\/\/findcare.hyperstage.com.au\/?job=care-manager-12-months-maternity-leave-contract","title":{"rendered":"Care Manager 12 months maternity leave contract"},"content":{"rendered":"<article>\n<p><strong>Deliver and develop tailored Care Plans for each client on the program and oversee the ongoing assessment of clients\u2019 needs and preferences within Home Care<\/strong><\/p>\n<ul>\n<li>12 months maternity leave contract<\/li>\n<li>Maximise your take-home pay with NFP salary packaging<\/li>\n<li>Enjoy a work-life balance with flexibility to work 2 days from home<\/li>\n<\/ul>\n<h2><strong>Care Manager &#8211; what&#8217;s involved in the role?<\/strong><\/h2>\n<p>We have 12 months maternity leave contract available, Monday- Friday in the HammondCare at Home Team <strong>Footscray<\/strong> Office, managing our clients during our growth phase.<\/p>\n<ul>\n<li>The Care Managers assist in providing services tailored to the needs of individual clients, delivered within budget and service expectations, including ongoing client assessment and the development of care plans in consultations with key stakeholders.<\/li>\n<li>Responsible for care management of existing clients and client-related KPIs<\/li>\n<li>Ensuring the care plans are up to date for our clients, and they\u2019re receiving an appropriate amount\/type of care that corresponds with their need, approval and funding<\/li>\n<li>Provide professional and competent day-to-day delivery of services to clients in line with service expectations and work to ensure that staff have a respectable understanding of our individual clients\u2019 needs and preferences.<\/li>\n<\/ul>\n<h2><strong>What do <em>you<\/em> need to apply?<\/strong><\/h2>\n<ul>\n<li>Certificate IV in Aged Care &amp; a minimum of 2 years relevant industry experience<\/li>\n<li>Knowledge of the Home Care Packages Program, CHSP services, Aged Care Quality Standards and Commonwealth Home Support Programme<\/li>\n<li>Experienced in operating effectively in a multi-site environment<\/li>\n<li>A current driver\u2019s licence and willingness to travel between HammondCare\u2019s clients, sites, and services (salary package can include a packaged motor vehicle on request)<\/li>\n<li>Strong administrative and computer skills.<\/li>\n<\/ul>\n<h2><strong>About HammondCare<\/strong><\/h2>\n<p>HammondCare specialises in Aged and Dementia Care, Palliative Care, Rehabilitation, Mental Health services for older people, and other related Health and Aged Care services.
